The form uses the latest internet technology to make it quick
and easy for you to complete and submit an online application. The
form will help and guide you through your application and make sure
it is filled in correctly.

When the form has been completed and you have checked that it
is correct, pressing the 'Submit data' button will send the
data onto our secure server so that we
can begin processing your form immediately.

We need
to see original proof to confirm the information
you have told us about on the form. It is important that you
provide this proof as soon as possible. We are unable to pay you
without the proof that is requested at the end of this claim form.
Please ensure that you provide all the information requested as
insufficient proof will delay any payments to you. Take your proof
to one of the Customer Service Centres at Town Hall, Batchley,
Winyates or Woodrow.

Response
We have a Quick Response service
available. If you bring all the proof requested to the Customer
Service Centre at the Town Hall within 2 working days of submitting
your claim form, then we will be able to process your claim in the
next 2 working days.
This means that you will have your benefit
paid sooner. Remember these must be original documents, and all
proof must be handed in at the same time. The advisor will scan or
copy your documents and hand them back to you, along with a
receipt.
We also have a home visit service for people unable to
visit the Customer Service Centres (either through illness or
