How to get helpYou can ask for help yourself or you may ask someone such as a relative, friend or health professional to contact us on your behalf. In the first instance contact should be made with the Adult Duty Team or a Customer Services Advisor at our Access Centre on 0845 607 2000.
We will ask for some basic information about you. This is an initial assessment, as we need to know more about you to decide if we can offer you a service.
A Community Care Assessment usually takes place to identify what your needs are. We will ask you and your carers to be involved in this as well as other professionals who may know you.
There is no charge for seeing a social worker but many of the services we can arrange may require a financial contribution. This will be explained to you at the time of assessment.
